You will find a website listed on the back of your BCBS card.  By going to this website your will find detailed information about your vision plans. There may also be an 800 number listed and by calling, the service rep should give you detailed information as well.

Once you have your plan details, contact our office and we will then be able to contact Blue Cross Blue Shield and verify your coverage.

If you desire to make an appointment to be evaluated for a “medical condition” and you have an “HMO” Blue Cross/Blue Shield plan, you will need a referral from your PCP (Primary Care Physician) prior to making an appointment with our office. Please contact your PCP (Primary Care Physician), who will then initiate the process for your appointment.
