Will you be able to tell me what my payment responsibility will be at the time of the exam?
Yes, Davis Vision will provide you with an Identification Number. Once you have given us that information we can verify the amount you will be responsible for. We can also look up your Davis Vision plan with Primary Member's Name and Date of Birth.

I know I have Davis Vision but they did not give me a card. What will you need to verify my coverage?
Davis will allow us to verify your benefits using the Primary insured's social security number. In some cases we can verify benefits using the ID# number printed on your Blue Cross Blue Shield card.

In some cases yes, Davis Vision can be affiliated with your medical plan.
For you to determine your Vision plan , we recomend that you log in to the website or call the phone number on the back of the insurance card that was provided to you.
