Yes, Safeguard Vision should have provided you with a member ID card. Once we have verified your information we can provide you with the correct amount of your Co-Pay.

What if I was not given a member card with my ID number on it?
If Safeguard Vision did not provide you with a member ID card, please provide us with your Primary insured's name and date of birth.
We can then contact Safeguard Vision and ask for verification of your coverage.
