VSP does not always give a member ID card.   VSP uses the last four numbers of the primary insured's social security number, name and date of birth to verify coverage.

We can then contact VSP and ask for verification of your coverage.   

 *If we are verifying for a dependant will also need their Name and date of birth. *

In some cases yes, VSP will be affiliated with your medical plan.
